An online claims form is located on our website. Please submit this form to receive a credit or replacement for any damages. If you need a replacement that has to go by pallet, it will ship with your next pallet order

Forty West Designs strives to provide our clients with high quality merchandise shipped with carrier-approved materials. We understand damages and defects may occur and we apologize for the inconvenience.

Please inspect your merchandise immediately BEFORE SIGNING FOR YOUR SHIPMENT. It is your responsibility to note any damage to cartons on all copies of the freight bill. This must be signed by the driver and the consignee. Forty West Designs is NOT RESPONSIBLE FOR FREIGHT DAMAGE THAT IS NOT NOTED ON THE FREIGHT BILL.

For concealed shipping damage, you must keep the carton and packing material and call the carrier within 7 days of delivery to request an inspection. Do not remove merchandise from premises before inspection is made and retain a copy of the inspection report.

DEFECTIVE MERCHANDISE CLAIMS MUST BE RECEIVED WITHIN 72 HOURS OF RECEIVING YOUR SHIPMENT. CLAIMS NOT REPORTED WITHIN THIS TIME FRAME WILL NOT BE ACCEPTED. No exceptions.

Please note: Photos are required for damage claim submission. Photos of the damage and of any noticeable damage to the inner/outer packaging or cartons are REQUIRED in order for claims to be processed.

Forty West Designs must authorize all adjustments, deductions, or chargeback’s. A customer service representative will contact you within 7 business days once your damage claim form has been submitted.
